The Surplus and Transfer Form is required for removing any NSCC Property that is assigned to you.
The Surplus and Transfer Form is designed for you to be able to type in all information. Please type in all information that you have on the items that you are surplusing. This allows the Property Manager to be able to read the form and make sure the correct item(s) is/are picked up. Please mark if the form is for Surplus or Transfer.
Fill in the Name, Phone Number, Date and Department.
If there is an NSCC Tag# on the item it needs to be on the form: The tag will either be a State tag (metal) or an NSSC tag (sticker) that begins with NT, NS, or for older tags C, F, or P. DO NOT REMOVE THE TAG! The person that picks up the item(s) will need it to verify that the correct item(s) was/were picked up.
If the Serial# is known that can be placed on the form as well.
Under the Description, please give as much information as you can. This helps when selling the item(s), as buyers usually want to know as much as possible on the item(s).
Current Location of the item(s). Please make sure that all items show where they are located. Multiple locations can be used on the form as long as it is the same department.
Condition of the item(s). Check what condition the item(s) is/are in. This helps with selling the item(s).
